MVV
As of 5 of April 2010, if you apply for an MVV (temporary residence permit) you need to fill out the MVV application form.

Here follows information about and the requirements for a temporary residence permit (MVV) also called a long stay visa.
A MVV (D) visa is required if a person plans to stay in the Netherlands for more than 90 days within 6 months, for instance for the purpose of work, study, living with a partner etc. This visa can be used during 3 months after issuing. In order to apply for your residence permit (VTV) for the Netherlands, you must present yourself to the municipality where you are going to stay within 8 days upon arrival. Processing of the application of the VTV may take several months.
Please remember that a MVV (D) visa itself does not mean permission to reside and is valid only to enter the Netherlands to obtain a residence permit. You can use this visa to travel in the Schengen area, you need to have a proof of the purpose of trip when travelling.
Every person applying for a MVV (D) visa should present in all occasions:

two recent identical passport-size color photographs (one glued to the application form, another attached by a paper-clip): size photo: 35 x 45 mm; background: white, light blue, light grey; head: uncovered, face and eyes fully visible; position: looking straight into the camera; expression: neutral, mouth closed; spectacles: eyes fully visible, no reflection and /or shadow; exposure: even.
If you are traveling with a child who is mentioned in the passport, a separate form should be presented for it with one photograph of that child attached to the form and one equal separate.

original passport and photocopies of all marked pages; As of 5 of April 2010 at the momen you apply for a visa, your passport may not be older than 10 years. Even when the passport is valid for more several years, the passport cannot be accepted, when it is older than 10 years. The passport should contain for each visa at least 2 empty pages, 1 page for the visa sticker and 1 for the entry stamps

documents relating to the purpose of your stay.
Migrants wishing to settle in the Netherlands for, in particular, the purposes of marrying or forming a relationship will be required to take the civic integration examination abroad. An application for a MVV can only be processed with a positive result of this civic integration exam. Click here to obtain more information.

Please take notice of the next important details:

a MVV application cannot be processed until notification is received from the IND (Immigration and Naturalization Service) in the Netherlands that the corresponding fee has been paid (see IND-website). After submitting the application three weeks are accorded to do the payment;

the application will not be processed while it does not contain all required documents. After acceptation of an incomplete application a maximum of three weeks will be allowed to deliver the missing documents. After three weeks, even if the missing documents have not been delivered, the application will be sent to the IND. Most probably this will result in a refusal of the application.

if necessary, the applicant can be asked to provide additional information and inquiries can be made.
The visa is one of the conditions to get free access to the Schengen area. When crossing the border you may be asked to supply the controlling officer with original documents that provide proof of your financial means, of insurance against costs for medical treatment or repatriation, of the purpose of your travel, length of stay, etc.
